ELIGIBILITY CRITERIA FOR SUBSIDIZED CARE FOR UNINSURED OR UNDERINSURED PATIENTS

The Samuel J. Friedman Health Center for the Performing Arts is opening in March 2017. Here is more information for those with high deductibles or who are uninsured to explain how we can continue to provide your primary care.

If you are uninsured, or have a plan with a deductible of at least $2,000 for primary care, and you are a member of the entertainment industry, you may be eligible for an Actors Fund subsidy for up to three primary care visits per year; The Actors Fund will subsidize the cost of your office visit, and you will be charged a $40 copay per visit. In order to qualify for this subsidy, you must meet all of the following criteria:

  1. Be a member of the performing arts and entertainment industry:
  • Entertainment industry earnings of $3,000 a year for three out of the last five years or
  • $5,000 a year for 10 out of the last 20 years
  1. Have documented annual income between $23,540 and $60,000.

Note: if your income is below $23,540 and you live in New York, you are eligible for comprehensive coverage through Medicaid or the Essential Plan. If you live outside of New York, please contact us. If your income is above $60,000, you will be charged the full cost of the visit.

If you require more than three visits per calendar year, additional medical and financial need requirements will apply.

Meet Our New Doctors


Dr. Ramon Pesigan

Dr. Ramon Pesigan attended medical school at the University of the Philippines, and graduated from an Internal Medicine residency at St. Luke’s-Roosevelt Hospital Center in New York City. He also received further training in Primary Care Sports Medicine after completing his fellowship at Atlantic Sports Health in Morristown, New Jersey. He most recently became a primary care and sports medicine practitioner with Mount Sinai Health Services. He specializes in Primary Care and non-operative care of sports injuries; this includes Preventative Sports Medicine, Nutrition, Integrative Medicine, and ultrasound-guided injections. In his free time, he enjoys spending time with his family, learning about the world through travel, and is an avid soccer player.

Dr. Jason Kindt

Although having a passion for the theater and participating in many high school productions, Dr. Jason Kindt quickly realized that his enthusiasm for musical theater does not make up for his lack of pitch.  So, he went to medical school.  He graduated from Ursinus College in 1995 and Philadelphia College of Osteopathic Medicine in 1999.  He is board certified in Family Medicine and worked for the Reading Health System in Hamburg, PA, where he was voted “best family practice” five years in a row, until 2013.  After that he had to be closer to Broadway so he moved to New York City and has been doing Urgent Care in midtown Manhattan ever since, leaving him plenty of opportunity to catch all the shows in town.  Dr. Kindt is a theater person at heart and is very excited to be working with this community.  He is a loving and caring family physician who is dedicated to helping his patients thrive in every aspect of their lives.  “If art is to nourish the roots of our culture, society must set the artist free to follow his vision wherever it takes him.” (John F. Kennedy)
